If you are concerned about an employee member of our campus community, share the nature of that concern with our Human Resources and Talent Management (HRTM) Office by submitting this form. Please be aware that a ºÚÁÏÍø HRTM staff person will likely reach out to the member of our campus community whom you have reported to offer support and assistance, typically within one to two business days. Should you wish for this not to occur, please be very clear about this in your report. However, in the event that there is a determination of a significant health and safety risk either to the employee or to the campus community, there may be an obligation to follow up on concerns regardless of your intention. Someone may also follow up with you to gain additional information.
